A Geriatric Care Clinic is a community-based healthcare initiative that focuses on promoting healthy ageing through comprehensive health assessment, early detection of illnesses, health education, counselling, and appropriate referral services for older adults. It also ensures continuity of care through regular follow-up and health promotion activities, thereby improving the overall well-being and quality of life of the geriatric population.
The Department of Community Health Nursing and the NSS Unit of Pushpagiri College of Nursing, through the students of the 8th Semester B.Sc. Nursing, participated in the Geriatric Care Clinic conducted at Kuttoor Family Primary Health Centre on 21 July 2026, from 9:30 AM to 12:00 PM, as per the request of the Kuttoor Grama Panchayat.The Geriatric Care Clinic is an ongoing community-based healthcare initiative conducted twice a week to ensure continuity of care, regular follow-up, early identification of health problems, and health promotion among the geriatric population. As part of this initiative, the students actively participated in providing geriatric health services under the supervision and guidance of the healthcare team.The programme was carried out with the support and cooperation of the Medical Officer (MO), Health Inspector (HI), Junior Public Health Nurse (JPHN), ASHA workers, and other healthcare staff of Kuttoor Family Primary Health Centre, whose guidance and active participation contributed to the successful conduct of the clinic.
During the clinic, a detailed assessment was carried out for each beneficiary. This included the collection of identification data, present health history, past and personal history, followed by a comprehensive physical assessment. The physical examination included assessment of vital signs, anthropometric measurements (height, weight, and BMI where applicable), general physical examination, and systemic examination. In addition, functional assessment, nutritional assessment, and psychological assessment were performed to identify the overall health status and specific needs of the elderly.Based on the assessment findings, individualized health education and counseling were provided regarding medication adherence, balanced nutrition, physical activity, fall prevention, management of chronic illnesses, and the importance of regular follow-up. Beneficiaries requiring further evaluation or specialized care were referred to the Medical Officer.
The clinic is being successfully conducted with the active participation of the beneficiaries and the wholehearted support of the Primary Health Centre team. We express our sincere gratitude to the Panchayat and the staff of Kuttoor Primary Health Centre for providing us with this valuable learning opportunity. We also extend our heartfelt thanks to our faculty guides, Sr.Mayukha, Associate Professor, Department of Community Health Nursing, and Dr. Jeena Kunjachan Assistant Professor, Department of Community Health Nursing for their constant guidance, encouragement, and supervision throughout the programme, which greatly enriched our learning experience and contributed to the successful conduct of the clinic.
